The days of being offered a perverse incentive to prioritize volume over value are near extinct. Perpetuated by a value-based and consumer-driven market, we are challenged to design a care model that aligns with value-based payment model systems, which are increasingly replacing traditional fee-for-service and volume-baed payment models.
Care models must be patient-centered and guided by a team-based approach, enabling each team member to practice at their highest licensure and encouraging innovative non-traditional virtual and remote visits enabled through technological advances. These common sense change have proven to enhance access, financial strength, continuity, coordiantion, population health, self-management, and care management...reducing avoidable readmissions, complications, and admission to the ED.
And, let us not forget that patients are problem solvers. Embracing the patient as a member of the care team and activating them in their care as shared decison makers encourages patient accountability, adherence, experience, and quality.
